For Freshmen
- Form 137 (Permanent Student Record/SF10): Form 137 is different from your report card (Form 138). Please visit the Registrar’s Office to get the request letter to be submitted to your previous school’s Registrar.

- Original and photocopy of PSA Birth Certificate
- Original and photocopy of Certificate of Good Moral Character
For Transferees
- Certificate of Transfer Credential (Honorable Dismissal)
- Certificate of Good Moral Character (if not yet included in the Honorable Dismissal)
- Original Transcript of Records (TOR) with a notation “Copy for the National Teachers College”
- Original and photocopy of PSA Birth Certificate
- If records are in maiden name, original and photocopy of Marriage Certificate (if records are in maiden name)

Step 4: Answer the Medical Profile
BLDG refers to the building where your classroom is located. Kindly login to your Student Portal for the campus map.
- MDL: Mendiola Campus
- ADM: Admin Building
- ELEM: Elementary Building
- INF: Infantado Building
- SCI: Science Building
- YLA: Ylagan Building

SET / SECTION refers to your section and determines wheter the subject is cyclical or semestral
- The first number refers to your year level
- The digits after the dot (.) refers to your Block Number (section)
- CY 1 at the end indicates that the subject is in Cycle 1. Classes for Cycle 1 subjects will start on: to be announced
- CY 2 at the end indicates that the subject is in Cycle 2. Classes for Cycle 2 subjects will start on: to be announced
- SEM at the end indicates that the subject is SEMESTRAL. Classes for Semestral subjects will start on: to be announced

SCHEDULE refers to the time of your classes
- WS: your classes are every Wednesday and Saturday
- MTH: your classes are every Monday and Thursday
- TF: Your classes are every Tuesday and Friday
